If you are new to our office, please book a new patient exam. If you are booking for your child, please use their name when booking the appointment. Once you book your appointment, intake forms will be sent to your email. Please have these filled out at least 1 hour prior to your appointment, so we can review your information before you come in. For functional medicine patients, we must have your intake forms filled out at least 24 hours in advance to your initial appointment.

We are a self pay clinic, meaning we are not in network with any insurance companies and will not bill them. Because we are out of network with all insurance companies, due to federal laws, we are not allowed to see anyone with federal insurance benefits, such as Medicare, Medicaid, Medical Assistance, and Tricare, but we would be more than happy to refer you to a provider that does accept your insurance.

Our office is located in Navarre/Orono, although the address is “Wayzata”, in the Casco Run Offices.
